Here's how Care works... You Choose the Plan Best for You, Plan A or B.

SIX (6) IMPORTANT BENEFITS:

As the direct result of death caused by an accident to the Insured while the policy is in force, PLAN A or PLAN B pays the following benefits:

1. ACCIDENTAL DEATH BENEFIT

PLAN A:	$100,000 paid to the person you name as beneficiary on the application.

PLAN B:	$ 50,000 paid to the person you name as beneficiary on the application.

2. MONTHLY INCOME

PLAN A:	$1,000.00 paid to the beneficiary monthly for the rest of his or her life.*

PLAN B:	$ 500.00 paid to the beneficiary monthly for the rest of his or her life.*

* Note age limitations in the policy as follows:

3. LOW MONTHLY RATES

4. SUPPLEMENTAL COVERAGE - CHILDREN'S EDUCATION

PLAN A & B: College fund available by rider. This benefit pays up to $20,000 per year for four years to a minor dependent if the insured dies of an accident prior to the dependent's attained age 18. The benefit is paid out in four annual installments on or after attainment of age 18. * One unit ($10,000) or two units ($20,000) are allowed per child for up to four children per plan.

*Monthly rate per unit for each child is based on child's age at issue as follows: under age 6, $1.25; ages 6 through 12, $2.00; ages 13 through 17, $2.50.

5. GUARANTEED RENEWABLE TO AGE 70

Your policy cannot be canceled prior to your age 70 as long as premiums are paid on time. The premium may be adjusted, but only for all policyholders in your state.
