North Coast Life acts as transfer agent for its common and preferred stock. A transfer
request must include the original stock certificate (which either has been signed and
dated on the back or be accompanied by a signed and dated stock power) and instructions
advising the name, address and Social Security number of the person to whom the stock is
being transferred.
If the stock is being transferred because of death, the request must also include a
copy of the death certificate and documentation showing authority to request the transfer
(Power of Attorney, Executor of Estate, etc.)
If the original certificate cannot be located, an Affidavit of Loss and Indemnity
Agreement must be completed (Download
Affidavit and Indemnity Agreement). This form is
viewable and printable using Adobe's Acrobat Reader. Print the form, fill in the
pertinent information, have the form notarized and return it to North Coast Life by mail to the attention
